Taking social histories and making social diagnoses

Individual counselling, groups and lectures on the following topics

- Social welfare law

- Financial security

- Vocational rehabilitation (MBOR, phased return to work, benefits for participation in working life, pension for reduced earning capacity)

- Home care for those requiring care and support, applying for care level (Pflegegrad)

- Aftercare: rehabilitative exercise (Rehasport), where applicable T-RENA/IRENA

Participation in interdisciplinary team meetings

Collaboration with external counselling agencies

Your profile / Your values:

Diploma, Bachelors or Masters degree in Social Work/Social Pedagogy

Sound knowledge of social legislation and networking

Patient-centred approach, team-working skills and interpersonal competence

Counselling skills, flexibility and the ability to work independently

Your responsibilities:

Medical care and treatment of our patients within an interdisciplinary team

Prescribing medical therapy components and close interdisciplinary cooperation with nursing, physiotherapy, occupational therapy and psychotherapy to implement an individual treatment plan

Admission, interim and discharge examinations

Participation in medical meetings, ward rounds and on-call duties

Contributing to the conceptual development of the medical service

Where applicable: sonography, duplex sonography, echocardiography

Your profile:

Completed medical degree with German licence to practise (Approbation)

You are a specialist in Internal Medicine, General Practice or Physical and Rehabilitative Medicine, or a resident in training

Enjoy working in a multiprofessional team

Sense of responsibility, strong communication skills and empathy

Interest in patient-centred, holistic care
